Title IX Complaint Form

Reports to the Title IX Coordinator can be made via email, phone, in person, and anonymously at the contact information below.

Individuals who have experienced or witnessed an alleged incident of sex discrimination or sexual misconduct have many options. They include:

• Consultation with a confidential resource or other support organizations, 
• Requesting supportive measures, and 
• — internal and/or criminal.

Deciding among these options can be difficult. Individuals are encouraged to seek assistance from a confidential resource before deciding how to proceed.
